Welcome to our medical practice. As I am sure you are aware, our healthcare system continues to undergo drastic changes. As you may already know, our medical system is in constant flux. With insurance policies with rules as complex as our tax code, many physicians have retired or changed careers in frustration. In my life as a physician, I have been fortunate to train at a time when the doctor was both patient advocate and a friend. I have been able to practice at a time which saw many changes, mostly for the worse in terms of the doctor-patient relationship. Corporate medicine, insurance companies and our government have put a chasm between the doctor and patient resulting in impersonal care and frustration for both the patient and the physician. When it comes to medical care, we need a reliable place to come for advice and truth. What is lost in today’s post pandemic world is the art of medicine. Practicing medicine is a calling, a vocation. The changes in our medical system has cut the heart out of the doctor and patient relationship.
With great faith in God, the ultimate physician, and a thorough understanding of the importance of the doctor-patient relationship in pursuit of truth and trust, We feel that we provide the best care that can be offered without interference of corporate medicine and government intrusion. We are dedicated to the pursuit of truth and well-being of our patients, first and foremost. Precisely, this type of care is arranged to better serve you while revitalizing the idea of the traditional family doctor.
Our medical practice is a Membership practice. The extent of services offered when enrolled in the Premier Membership practice include same day office visits when needed, (within 48 hours if not emergent), telemedicine visits, telephone triage when the problem can be managed this way, onsite phlebotomy services with next day results, timely prescription services, personally supervised care when hospitalized, expanded well-physical exams as well as the unrivaled coordination of care that has been the hallmark of our medical practice and office staff for years. Dr. D'Agostino has admitting privileges and coverage at all local hospitals.
If you are interested, contact our office and we will forward a contract for the Premier Membership. The membership fee is $1000 which is billed annually. Payment plans can also be arranged. There is a discount for couples and additional family members under 25. Please note that membership fees cover only the non-covered services outlined in the contract, and you will still need health insurance and Medicare to cover for office visits, other examinations, specialty physicians, lab tests, and hospitalizations.

We accept most public and private insurance plans. We also offer special pricing for cash-paying patients for most procedures. We do not accept any HMO plans or Blue Select Plans and we are out of network with United Health Care.

Anthony M. D'Agostino, M.D., FACP is a graduate of the University of Notre Dame with a BS in Biochemistry and a Graduate of the University of South Florida College of Medicine with an M.D. in 1992. He was a resident in Internal Medicine from 1992-1995. He served as Chief Resident in Internal Medicine at the James A. Haley VA in Tampa, Florida from 1995-96 and started a solo practice in Internal Medicine and Primary Care Medicine in his hometown of Naples, Florida in 1996. He has been in practice since then and has over 2,500 active patients. His office is a throwback to the past and serves as a "Medical Home" for all of his patients. In his practice, you will experience the personal relationship beckoning to the golden age of medicine. Our practice was built through word of mouth, as it should be.
